All
NEW student athletes (freshman and transfers) need to
have a complete Physical Examination on file with UCI
Sports Medicine prior to obtaining clearance for participation.
All Physicals must be current (within 90 days of the
start of the academic term). The Physical Examination
form is available for download.
Prior
to obtaining your physical, you should fill out your
health history questionnaires and insurance information
sheets. Your Entrance Physical may be obtained one of
two ways:
- You
may have your personal physician; HMO, etc. perform
the physical on the form provided and send the completed
information to UCI Sports Medicine at the address
provided below.

You may schedule a physical at Student Health Services
(SHS) at (949) 824-5304. The physical will cost $90
and will require two days to complete in order to
check TB test results and follow-up on any lab work.
This will need to be scheduled prior to August 1st,
2006. If you opt to have your physical done at Student
Health, you must get a copy of your history and physical
from Student Health and bring it to the Sports Medicine
Center.
PLEASE
Schedule your physical examination TODAY
to insure your completion of the physical on time so
that you will have no delays with your start date.
All
students are required to have a proof of immunization
on file at Student Health Services as a part of their
academic admission to UCI. We will photocopy the SHS
Immunization / Physical Exam forms and submit it
to Student Health Services for you. This exam is a “one
time” exam (unless you leave UCI for more than
two quarters) and will allow you to use SHS during your
entire academic career at UCI.
NOTE:
Student-Athletes are responsible for all costs associated
with the Entrance Physical Exam!
All NEW and RETURNING student athletes must undergo
an Athletic Medical Screen
and Clearance. This is an annual process, which
is in compliance with the NCAA Sports Medicine guidelines.
The “Screen and Clearance” will be performed
by the UCI Intercollegiate Athletics Team Physicians
and will take place at the Gottschalk Medical Plaza
located on campus (at Academy Way and Medical Plaza
Drive). Clearances will be performed on specified dates
from 8:00 am to 12:00 pm. Each sport will be assigned
a date and time for their “Screen and Clearance”
which corresponds to their Fall practice start date.
If
you have a complex medical history or if you have had
a recent orthopedic problem, you must bring in a copy
of medical records documenting any prior athletic injury
for our physicians to review before you will be cleared
to participate.
ALL
medical forms must be turned in to UCI Sports Medicine
by AUGUST 1st 2007.
If
you have your physical done at UC Irvine Student Health
Services you must obtain a copy of the SHS Health History
and Physical Forms from Student Health Services and
hand carry them to UC Irvine Sports Medicine at Crawford
Hall..
